APPLICATION:
The application is submitted electronically by the student: SEE BELOW.
REFERENCES:
A student’s application must be accompanied by three reference forms emailed to the Theriogenology Foundation by the references themselves. The student applicant does not submit the reference forms. The applicant should become familiar with the format of the reference forms and then forward them to their three chosen references for their separate submission.
- At least one of the three references must be a member of the Society for Theriogenology and/or a Diplomate of the American College of Theriogenologists.
- One reference form must be from a faculty member at the student’s current veterinary medical academic institution. They should use this format: LINK TO FACULTY REFERENCE FORM
- Two additional reference forms must be from two other persons who know the applicant (e.g., a friend, former teacher, clergyman, employer, family member, other past or present university or veterinary school faculty member, etc.). They should use this format: LINK TO ADDITIONAL REFERENCE FORM
- The reference forms are confidential. They will not be shared by The Theriogenology Foundation with the applicant or anyone else. However, the reference may share it with the applicant at his/her discretion.
- The three reference forms are a critical part of the application. Each reference form comprises 10% of a reviewer’s score for each application, therefore the three reference forms comprise 30% of the total score.
BY NO LATER THAN MIDNIGHT, MONDAY, FEBRUARY 13, 2023, SUBMIT THE FOLLOWING COMPLETED APPLICATION. APPLICANTS WHOSE THREE REFERENCES HAVE NOT BEEN SEPARAELY EMAILED TO TF BY THIS SAME DEADLINE WILL BE DISQUALIFIED.
